As of Monday, November 23, masks must be worn ON ICE by all players/coaches, etc.
De Pere Youth Hockey utilizes both rinks for games and practices, so it is important to know the rules of each rink as you will most likely play at both during the season.
The DPYH Board has been working through the summer to ensure we can safely play hockey this year and has developed a COVID-19 Policy. Please help us keep our local rinks open and our players on the ice by following all the safety protocols of the Policy, most importantly –
As part of the COVID-19 Policy, DPYH will be utilizing a flow chart, COVID-19 Exposure Protocol, to determine if a player or coach needs to be removed from participation and for how long. The flow chart addresses, among other things, how to handle close contacts, symptoms, and positive or negative results. Reporting of a close contact or of a positive test result must be completed within 24 hours of receiving the information. Additionally, reporting will need be given verbally or through electronic communication to the Head Coach and Team Manager and in writing via the DPYH COVID-19 Reporting Form available on this page. DPYH will be fully cooperative with the local health departments if contact tracing is needed.
It’s been a long road, but we finally made it; let’s get back to hockey!
The purpose of this reporting form is to communicate all close contact and positive Covid-19 results to help minimize the spread and continue with a successful 2020-2021 DPYH season.
Either before or after filling out this form, PLEASE be sure to contact your team manager immediately and do not attend any practices or games until further communication and decisions are made.
Please fill out one form per Skater if there are multiple siblings affected.
